Boxer urine problems

Please help. I have spent about $1,000 trying to figure out what is wrong with my boxer. She's a 3 1/2 year old fawn and lately has had UTI issues. She is now on a strict diet. The other night, she was laying down sleeping and urinated on the floor, I think it shocked her just as much me. what can be causing this for her? Should I have the vet check for anything specific? She's has an ultra sound done and numerous urine samples done. Please help!
