The only other thing to try is "Atopica" which your vet can order if he/she does not carry it. (There are also allergy shots but only about 60% effective)
This drug works miracles for some dogs, but it is not the be-all/end-all of therapy. The dog will still have occasional flare-ups that need to be managed with all the drugs you just mentioned. But it sounds like your dog would be a good candidate since all else has failed. You have to give it at least 30 days. And your dog should be tested for pre-existing kidney or liver disease prior to starting.
Granted, the drug is expensive, but if it works, it is well worth the cost. Plus think you much you spend on all the stuff that doesn't work...
